Intelligence and integration of care for smarter chronic wounds management.

We are a research project funded under the THCS Joint Transnational Call
(JTC2023) for collaborative research projects on “Healthcare of the Future”.

ICAREWOUNDS project will develop an Integrated Care model for chronic wound management.

Chronic wounds are a major concern for the healthcare system as their financial burden is estimated at 5% of total expenditure. The management is very complex, and the presence of different tissues, healing levels and factors such as the patient’s skin type determine the clinical strategy and wound dressings to use.

The problem

Despite there being great experts on chronic wounds, these are often followed-up by health professionals at primary care with a limited ability to make the most appropriate clinical decisions, which leads to a great variability in the management of chronic wounds and makes recovery harder, longer, and remarkably more costly.

The solution

With a consortium made up by Gradiant, Instituto de Investigación Sanitaria Galicia Sur, LUT University, Medical University of Lodz, Sigoria Ltd, University College of Dublin and CyberEthics Lab., the main aim of ICAREWOUNDS is to design, deploy, implement, and validate an Integrated Care model that facilitates the sustainability and efficiency of chronic wound management.

The consortium

Gradiant is a private ICT technology centre focused on the needs of industry and specialising in the following technologies: cybersecurity, artificial intelligence, advanced communications (5G-6G), quantum technologies, drones, and smart industry. An innovation provider, with nearly fifteen years of experience in technology incubation, more than 200 professionals in its team and with a presence in 30 countries and more than 400 customers.

In recent years, Gradiant has quintupled the licensing agreements of its innovation projects, which now reach 150 licenses in 18 countries. Recent international public references include Telefonica, Vodafone, Samsung, Stellantis, NATO, Indra, NTT Data, Babcock International and Boeing.

Cybersecurity has always been a strategic priority for Gradiant, as it is one of the essential technologies for digital transformation, along with connectivity and artificial intelligence. Gradiant leads ICAREWOUNDS aware of the transformative role of technological innovation in ehealth for both individuals and companies.

Transforming Health and Care Systems (THCS) Joint Transnational Call 2023 (JTC2023) “Healthcare of the Future”

Transforming Health and Care Systems (THCS) initiative has been established as a European partnership under Horizon Europe, co-funded by the European Commission. The aim of THCS is to coordinate and optimize research and innovation efforts in Europe and its partner countries supporting health and care systems transformation.

The objective of this call is to encourage the optimization of patient care pathways and contribute to the transition towards more:

The call addresses the challenge posed by the increasing number of patients treated in hospitals or other healthcare facilities and the need to ensure continuity of care and offer better integration of hospital services with outpatient care to ensure they are treated in the appropriate setting according to their respective medical condition in a healthcare continuum that makes the best use of resources and deliver better patient satisfaction. To reach this ambition, the complementarity of inpatient and outpatient care, and the coordination between them needs to be optimised.

ICAREWOUNDS project has received funding from the European Commission, ISCIII, NCBR, HRB, AKA and MUR  under the framework the co-fund partnership of Transforming Health and Care Systems, THCS, (GA N° 101095654 of the EU Horizon Europe Research and Innovation Programme (AC23_2/00048)
