ICAREWOUNDS: A New Approach to Chronic Wound Management

Chronic wounds remain one of the most persistent and complex challenges in modern medicine. From diabetic foot ulcers and pressure injuries to venous leg ulcers and post-surgical complications, clinicians face not only the biological complexity of impaired healing, but also systemic barriers: fragmented care pathways, limited continuity, and inconsistent decision-making across settings.

In this context, the European project ICAREWOUNDS introduces a new, integrated approach to chronic wound care — one that combines clinical expertise with digital innovation to support physicians in delivering more consistent, data-driven, and patient-centered treatment.

Why Chronic Wound Care Needs Rethinking

Despite advances in treatment methods, chronic wound management still often relies on:

  • subjective assessment,
  • variable clinical experience,
  • and limited access to longitudinal patient data.

Patients frequently move between primary care, specialist clinics, and hospital settings. At each stage, information may be incomplete, documentation inconsistent, and treatment strategies difficult to standardize. For physicians, this creates an environment where optimal decision-making becomes more difficult — and outcomes less predictable.

What ICAREWOUNDS Brings to Clinical Practice

ICAREWOUNDS addresses these challenges by developing an AI-supported, integrated care model specifically designed for chronic wound management.

At its core, the project focuses on supporting — not replacing — clinical judgment.

The solution aims to:

  • support more accurate and consistent wound assessment,
  • enable structured and standardized clinical decision-making,
  • improve communication between levels of care,
  • facilitate personalized treatment pathways,
  • and ultimately reduce healing time and complications.

By integrating data from multiple sources and applying trustworthy AI models, ICAREWOUNDS helps physicians navigate complex cases with greater confidence and clarity.

From Fragmented Care to Integrated Pathways

One of the most critical aspects of the project is its emphasis on continuity of care.

Rather than functioning as a standalone tool, ICAREWOUNDS is designed to fit into real clinical workflows and healthcare systems. It leverages interoperability standards (such as FHIR) to ensure that patient data can be shared securely and effectively across different providers and settings.

This shift — from isolated interventions to coordinated care pathways — has the potential to significantly improve both clinical outcomes and system efficiency.

Evidence, Not Assumptions

Importantly, ICAREWOUNDS is not just a conceptual framework. The solution is currently being evaluated in multicenter clinical studies across several European countries, with active involvement of healthcare professionals.

This means that usability, clinical relevance, and workflow integration are being tested in real-world conditions — a crucial factor for any digital health innovation aiming for meaningful adoption.

Supporting Physicians in a Changing Healthcare Landscape

Healthcare systems across Europe are under increasing pressure: aging populations, rising prevalence of chronic diseases, and limited clinical resources. Chronic wound care sits at the intersection of all these challenges.

ICAREWOUNDS represents a broader shift in medicine — toward tools that enhance clinical expertise, improve coordination, and make care more predictable and scalable.

For physicians, this is not about replacing experience with algorithms. It is about augmenting decision-making with better data, clearer pathways, and more consistent standards of care.

Looking Ahead

Chronic wounds will remain a significant clinical burden in the years to come. However, projects like ICAREWOUNDS demonstrate that with the right combination of medical knowledge, digital tools, and system-level thinking, meaningful improvement is within reach.

The question is no longer whether technology will play a role in wound care — but how well it can be integrated into everyday clinical practice to truly support those making decisions at the bedside.

The platform begins its study with healthcare professionals specialised in chronic wound management  As its first phase nears completion, the multicentre study will evaluate usability, trust and added value of a solution based on trustworthy AI and eHealth technologies  ICAREWOUNDS continues to advance towards its goal of transforming chronic wound care through digital technology and trustworthy Artificial Intelligence models.
