Led by Gradiant and involving seven organisations from five countries, this European project aims to transform the care provided to patients with chronic wounds
The initiative is progressing in the development of a digital platform that facilitates integrated care for patients using eHealth technologies and trustworthy Artificial Intelligence (AI), optimising the management and personalised treatment of these wounds, thereby improving both quality of life and the efficiency of healthcare systems

The international consortium of the European project ICAREWOUNDS held its third in-person meeting on 21 and 22 May in the city of Łódź (Poland). This event is a significant milestone in the progress of this initiative, co-funded by the European Commission with the support of national agencies such as the Instituto de Salud Carlos III. Its objective is to transform the care of patients with complex chronic wounds through an integrated, digital and person-centred care model.
Led by Gradiant, with contributions from seven stakeholders across five European countries, ICAREWOUNDS is developing an innovative digital platform that combines trustworthy Artificial Intelligence (AI) and eHealth technologies to optimise clinical management, personalised treatment and monitoring of chronic wounds.
During its first year of implementation, the project carried out a thorough analysis of chronic wound care processes in Spain, Ireland and Poland, identifying the capabilities, limitations and specific needs of each healthcare system. Based on these findings, the consortium has designed a standardised and integrated care model that goes beyond clinical treatment of wounds to include key factors such as hygiene, nutrition, and other elements that influence recovery.
This model serves as the base for the development of the ICAREWOUNDS platform, currently under construction. This digital tool will enable healthcare professionals to monitor the condition of chronic wounds in real time, actively manage care, receive personalised clinical recommendations for each patient through the use of AI, and share daily care tips with patients. The platform’s pilot trials are scheduled to begin at the end of 2025 led by Instituto de Investigación Sanitaria Galicia Sur (IISGS), University College Dublin (UCD) and the Medical University of Łódź (MUL).
One of the key innovations of ICAREWOUNDS lies in its personalised approach. The platform will not only provide tailored medical guidance for each case but will also enable professionals to offer recommendations on essential complementary practices to accelerate healing and prevent complications. This will help improve patients’ quality of life and significantly reduce the costs associated with these conditions.
Furthermore, the project has begun collaborating with healthcare professionals and patient associations, ensuring that the solution is developed in alignment with real-world needs. In the coming months, work will continue on training and refining the AI algorithms to improve the accuracy of recommendations, as well as integrating all technologies for final validation.
With €1.5 million in funding from the European Commission and support from national bodies such as the Instituto de Salud Carlos III, ICAREWOUNDS involves the participation of the Galicia Sur Health Research Institute (Spain), CyberEthics Lab (Italy), Medical University of Łódź (Poland), University College Dublin (Ireland), Lappeenranta-Lahti University of Technology LUT (Finland) and Sigoria Security Solutions Ltd (Poland).
