Integrated Care in ICAREWOUNDS
This is the philosophy behind the ICAREWOUNDS project.
Chronic wounds often require long-term follow-up, coordination between different professionals, and timely clinical decisions. To address these challenges, ICAREWOUNDS has developed integrated care models for four major wound types: pressure injuries, diabetic foot ulcers, venous leg ulcers and surgical wound dehiscence.
These models combine evidence-based clinical practice with multidisciplinary collaboration and patient-centred care. In addition, ICAREWOUNDS has developed an integrated digital platform that incorporates advanced technologies, including artificial intelligence-based clinical decision support, to assist healthcare professionals in wound assessment and management.
